From Interstate 91 South take Exit 48 (Route 220 - Elm Street) and go East past the shopping mall and commercial areas approximately 1.7 miles. Bear right when Route 220 splits to the left and continue on Elm Street to the first traffic light. Take a right turn on Elm Street at the light and Autumn Fields is straight ahead on the right.
From Interstate 91 North take Exit 47E (Route 190 - Hazard Avenue) and go East past the shopping and commercial areas approximately 2 miles to Elm Street. Take a left onto Elm Street and follow approximately .8 miles. Autumn Fields will be on the left across from the new Senior Center and Public Safety Complex.
